Risks of Blood Draw

I understand that blood collection involves minor risks, including but not limited to: bruising, bleeding, lightheadedness, and in rare cases, infection. I consent to the blood draw procedure.

Accuracy of Information

I confirm that all information I have provided is accurate and complete to the best of my knowledge. I understand that inaccurate information may affect the validity of test results.

Release of Information

I authorize TotalVirility to share my information with laboratory partners and phlebotomists as necessary to provide the requested services.
